Intravenous dexamethasone is the best prophylaxis against rebound pain after a nerve block

A network meta-analysis of 24 trials ranks intravenous dexamethasone first for preventing rebound pain and finds pre-emptive opioids probably worse than nothing, a quadratus lumborum block beats intrathecal morphine on early pain after Caesarean, and duloxetine's effect on arthroplasty pain is real but below the threshold anyone would notice.
