Four biologics beat placebo in palmoplantar psoriasis, on indirect comparison
A network meta-analysis for hard-to-treat palms and soles, what biologics do to mood in psoriasis, AI for vitiligo, and a trial of repeated switching between a ustekinumab biosimilar and its reference product.
The edition in brief
A Bayesian network meta-analysis of 20 randomised trials (2,158 patients) found that adalimumab, guselkumab 100 mg, ixekizumab and secukinumab beat placebo for palmoplantar psoriasis or pustulosis at weeks 14 to 16; ixekizumab ranked highest (SUCRA 87.2%), but the comparisons are indirect and the populations varied. A second network meta-analysis (eight studies, 4,319 patients) found that all biologic classes improved depression and anxiety symptoms against placebo in moderate to severe plaque psoriasis, with improvement in mood generally accompanying skin response. A systematic review of 54 studies of imaging-based AI for vitiligo reported diagnostic AUC above 0.90 and segmentation Dice 0.84, often matching experts, but datasets are small and not diverse. In a randomised double-blind trial of 453 patients, repeated switching between the ustekinumab biosimilar ABP 654 and the reference product gave similar exposure (AUCtau ratio 0.93, 90% CI 0.89 to 0.98), efficacy, antidrug antibodies and adverse events.
Palmoplantar psoriasis: adalimumab, guselkumab, ixekizumab and secukinumab beat placebo
Consider an IL-17, IL-23 or TNF inhibitor for refractory palmoplantar psoriasis, and do not choose between them on the SUCRA ranking alone.
Biologics and mood in plaque psoriasis: low mood and anxiety eased alongside skin
Screen for depression and anxiety in psoriasis, and expect mood to improve with biologic response without relying on it.
AI for vitiligo: high accuracy on curated images, but not yet ready for clinic
AI tools for vitiligo look accurate in studies, but ask about the evidence on your patients' skin types before relying on one.
Examine palms and soles before calling psoriasis 'mild'
Judge psoriasis severity by site and function as well as by body score.
Repeated switching between a ustekinumab biosimilar and its reference product showed no loss
Consider switching between this ustekinumab biosimilar and the reference product when needed; exposure and safety were similar across repeated switches.
