Natriuretic peptide screening found heart failure in one in four high-risk patients with diabetes
TARTAN-HF puts a number on how much heart failure sits unrecognised in the diabetes clinic. Also: a third of lab-confirmed diabetes goes uncoded or untreated, a structured self-management trial in people at risk of kidney disease, and a sterility recall of compounded GLP-1 injections.
The edition in brief
TARTAN-HF randomised 706 community adults with diabetes and at least one heart failure risk factor in the west of Scotland to NT-proBNP screening (echocardiography if 125 pg/mL or above) or usual care. At six months heart failure had been diagnosed in 24.6% of the screened group against 1% with usual care; the number needed to screen was 5, most cases had preserved ejection fraction, and SGLT2 inhibitor use rose from 24% to 39%. The trial measured diagnostic yield, not outcomes. A US claims study of 27,650 adults with laboratory-confirmed diabetes found only 66.6% had both a diagnosis code and drug treatment within a year; 9.6% had neither, and more comorbidity meant less follow-through. A 134-patient Chinese trial of a 12-week structured self-management programme in people with type 2 diabetes at high risk of kidney disease reported improvements in HbA1c, eGFR, systolic pressure and LDL cholesterol, but not albuminuria. FDA lists Class II recalls of compounded semaglutide and tirzepatide injections, some combined with cyanocobalamin, for lack of sterility assurance.
A third of laboratory-confirmed diabetes is not coded, not treated, or both
Treat a diagnostic-range HbA1c as a diagnosis to record and act on at that visit, especially in patients with many other conditions.
Structured self-management improved HbA1c and blood pressure, but not albuminuria, in people at risk of diabetic kidney disease
Consider a structured self-management programme for patients at high kidney risk, while continuing to treat albuminuria with drugs.
FDA lists sterility recalls of compounded semaglutide and tirzepatide injections
Check whether patients are using compounded GLP-1 injections, and treat these recalls as a reason to move them to a licensed product where possible.
In a patient with diabetes and breathlessness, think of preserved-ejection-fraction heart failure early
Ask the heart failure questions at every diabetes review, and use a natriuretic peptide as the first test when the answers are positive.
NT-proBNP screening found heart failure in one in four high-risk patients with diabetes
Consider natriuretic peptide testing in older patients with diabetes and another heart failure risk factor, with echocardiography if it is raised.
