The edition in brief
RESOLVE, an ICMR-funded trial in five Indian public hospitals, randomised 880 treatment-naive adults with non-cirrhotic chronic hepatitis C to 8 or 12 weeks of sofosbuvir-velpatasvir. Cure rates were 98.8% and 99.0% per protocol, and the shorter course was non-inferior on intention-to-treat analysis too, with no drug-related serious adverse events. ACHIEVE-4 compared oral orforglipron with insulin glargine in 2749 people with type 2 diabetes and cardiovascular or kidney disease: major cardiovascular events occurred in 4.2% and 5.0% (HR 0.84, 95% CI 0.59–1.20), meeting non-inferiority; gastrointestinal effects were far commoner with orforglipron and serious hypoglycaemia far commoner with glargine. In evERA, oral giredestrant plus everolimus extended progression-free survival over standard endocrine therapy plus everolimus in ER-positive, HER2-negative advanced breast cancer after a CDK4/6 inhibitor, most clearly in ESR1-mutated tumours (10.0 vs 5.5 months). A UK registry emulation of nearly 700 000 knee replacements found no difference in early revision between robotic and conventional surgery.
