Home blood pressure readings matched clinic checks for women at high pre-eclampsia risk
An Australian trial found no perinatal penalty and fewer admissions with remote monitoring, a review flags the contraception gap when GLP-1 drugs start, and ten years of uterus transplants in the US.
The edition in brief
Remote blood pressure monitoring for women at high risk of pre-eclampsia was non-inferior to specialist clinic checks in a 270-woman Australian randomised trial (April 2026): the perinatal composite was unchanged (HR 1.0, 95% CI 0.57-1.76), antenatal visits fell, and any-cause admission was less likely (HR 0.54, 0.30-0.97). A narrative review in AJOG argues that contraceptive counselling should be routine whenever a GLP-1 receptor agonist is started, because interaction data are thin and fertility may rise with weight loss. A Michigan case study of 17 hospitals found that lower first-birth caesarean rates went with stronger leadership, collaboration and structured improvement processes. The pooled US uterus transplant experience (60 transplants, 2016-2025) reported live birth or ongoing pregnancy in 70% of recipients, reoperation in 28%, and preterm birth in 58% of 48 babies. A meta-analysis of seven observational studies found nominal associations between some maternal cytokines and offspring autism, but no robust biomarker. Pearl: aspirin prophylaxis for high pre-eclampsia risk works only if it starts early.
Starting a GLP-1 drug in a woman of reproductive age is a contraception conversation
Make contraception part of every GLP-1 receptor agonist start in a woman who could conceive.
What separates hospitals with low first-birth caesarean rates
Diagnose why your unit's first-birth caesarean rate is high before choosing a fix.
Ten years of uterus transplantation in the United States: births in 70%, reoperation in 28%
Use these pooled figures when counselling women with absolute uterine factor infertility, including the donor's surgical risk.
Maternal cytokines and autism: signals, but no usable biomarker
Maternal cytokine levels are a research signal for autism, not a test to offer.
Aspirin for pre-eclampsia risk only works if it starts early
Identify high pre-eclampsia risk at booking and start aspirin before 16 weeks.
Remote blood pressure monitoring was non-inferior to clinic checks in high-risk pregnancy
Consider validated home blood pressure monitoring with specialist review to replace some surveillance visits for women at high pre-eclampsia risk.
