After osimertinib fails, test for MET: a combination extended survival

In SAFFRON, osimertinib plus savolitinib beat platinum-pemetrexed on progression and overall survival in MET-driven resistance. Also today: immunotherapy added to short-course radiotherapy in rectal cancer, and an antibody–drug conjugate replacing chemotherapy in PD-L1-negative lung cancer.

The edition in brief

In the phase 3 SAFFRON trial, 338 patients with EGFR-mutated NSCLC progressing on osimertinib, with MET overexpression or amplification, were randomised to osimertinib plus savolitinib or platinum-pemetrexed; progression-free survival (HR 0.55) and overall survival (HR 0.68; 12-month 70% vs 53%) both improved, with similar severe adverse event rates and frequent peripheral oedema. In UNION, 231 Chinese patients with locally advanced rectal cancer received short-course radiotherapy followed by camrelizumab and CAPOX, or long-course chemoradiotherapy; at interim analysis 3-year event-free survival was 91.3% versus 77.4% (HR 0.33), though the comparator was not a modern total neoadjuvant regimen. In OptiTROP-Lung06, sacituzumab tirumotecan plus pembrolizumab roughly tripled median progression-free survival over chemotherapy plus pembrolizumab (15.0 vs 5.7 months; HR 0.49) in 455 patients with PD-L1-negative non-squamous NSCLC, with fewer severe adverse events; overall survival is immature.
