Tarlatamab added to durvalumab maintenance lengthened survival in extensive-stage small-cell lung cancer

DeLLphi-305 reported median overall survival of 21.1 months against 12.7 with durvalumab alone. Also: perioperative durvalumab with enfortumab vedotin in cisplatin-ineligible bladder cancer, divarasib against first-generation KRAS G12C inhibitors, and magnesium with weekly cisplatin.

The edition in brief

Today's oncology edition closes on DeLLphi-305, an open-label phase 3 trial of 563 patients with extensive-stage small-cell lung cancer free of progression after chemoimmunotherapy. Adding tarlatamab to durvalumab maintenance gave median overall survival of 21.1 months against 12.7 months (hazard ratio 0.42, 95% CI 0.32 to 0.54) at interim analysis, with cytokine release syndrome, mostly grade 1–2, in nearly half. VOLGA randomised 695 cisplatin-ineligible patients with muscle-invasive bladder cancer: perioperative durvalumab with three cycles of neoadjuvant enfortumab vedotin raised 24-month event-free survival to 63.5% from 45.1% with cystectomy alone, and improved overall survival (hazard ratio 0.58). Krascendo 1 found divarasib extended progression-free survival to 9.8 months from 5.7 with sotorasib or adagrasib, and overall survival to 17.7 from 12.0 months. A 100-patient trial with weekly cisplatin and radiotherapy in head and neck cancer found intravenous magnesium lowered urinary markers of kidney injury. The pearl covers kidney and electrolyte checks on weekly cisplatin.

Cisplatin-ineligible bladder cancer: durvalumab with enfortumab vedotin around cystectomy improved survival

For cisplatin-ineligible muscle-invasive bladder cancer, perioperative durvalumab with neoadjuvant enfortumab vedotin is an option to discuss, with an overall survival benefit over cystectomy alone.

Magnesium with weekly cisplatin lowered urinary markers of kidney injury

Magnesium with weekly cisplatin reduced kidney injury biomarkers in this small trial; keep supplementing and monitoring, without expecting it to prevent all nephrotoxicity.

Extensive-stage small-cell lung cancer: tarlatamab plus durvalumab maintenance cut deaths by more than half

For extensive-stage small-cell lung cancer without progression after chemoimmunotherapy, tarlatamab plus durvalumab maintenance is an option to discuss, with a large survival gain at interim analysis.
