Periprosthetic hip fracture: the 48-hour rule may not carry over

A target trial emulation finds no mortality gain from surgery within two days for periprosthetic hip fractures, while ankylosing spondylitis neck fractures, renal cell metastases and pre-arthroplasty education get their evidence checked.

The edition in brief

A target trial emulation of 4,564 Japanese patients with periprosthetic hip fracture found no clear difference in 30- or 90-day mortality between surgery within two days and surgery on days 3-10 (90-day risk 2.35% vs 2.31%), suggesting the native hip fracture 48-hour standard should not be applied to these fractures purely to reduce short-term deaths - though avoidable delay is still not justified. A meta-analysis of 18 retrospective studies of cervical fractures in ankylosing spondylitis linked the anterior approach to about three times the complications of the posterior approach, with no difference in neurological recovery or mortality. A single-centre series of renal cell carcinoma long-bone metastases found similar reoperation rates after intralesional surgery and resection, with higher mortality after intralesional surgery that was not seen for solitary metastases. A small randomised trial found adding a live session or an app to booklet education before hip replacement changed neither length of stay nor satisfaction. The pearl covers neck pain after minor trauma in ankylosing spondylitis.

Cervical fractures in ankylosing spondylitis: posterior fixation had fewer complications

For ankylosing spondylitis cervical fractures, consider posterior fixation first where the fracture allows - complications were lower and neurological outcomes similar.

Renal cell metastases in long bones: intralesional surgery did not mean more reoperations

Intralesional surgery for renal cell long-bone metastases did not raise reoperation rates; reserve resection mainly for solitary lesions in fit patients.

An app or live session before hip replacement added little to a booklet

A good booklet appears to do most of the work; added apps or live sessions did not change stay or satisfaction before hip replacement.

Neck pain after a minor fall in ankylosing spondylitis is a fracture until proven otherwise

In ankylosing spondylitis, a minor fall with new neck pain needs CT and immobilisation in the patient's own posture.

Early surgery for periprosthetic hip fracture did not lower short-term mortality

For periprosthetic hip fractures, consider taking the time needed for revision-level planning; a few days' wait was not linked to higher short-term mortality.
