Rifapentine barely reaches breast milk, and hydroxychloroquine's metabolite may track lupus control better than the parent drug

IMPAACT 2001 modelling puts the infant's rifapentine dose through breast milk below 1% of the mother's, supporting weekly TB preventive therapy while breastfeeding. In lupus, CYP2D6 genotype set exposure to the active hydroxychloroquine metabolite linked to disease control.

The edition in brief

Pharmacokinetic modelling of the IMPAACT 2001 study of once-weekly isoniazid plus rifapentine in pregnant and postpartum women found breast milk concentrations of rifapentine about 2% of maternal plasma and a relative infant dose under 1%; the drug crosses the placenta at levels unlikely to matter. In 102 Japanese patients with lupus on hydroxychloroquine, CYP2D6 intermediate metabolisers made less of the active metabolite desethylhydroxychloroquine, and higher metabolite levels, not parent drug levels, were associated with low disease activity (adjusted OR 1.80 per SD); discrimination was modest. A review and FAERS analysis of bispecific antibodies in haematological cancers found teclistamab and glofitamab carried the heaviest toxicity and non-relapse mortality signals, mainly infection-driven, and mosunetuzumab the lightest. A case with drug levels showed lenacapavir concentrations stayed therapeutic alongside low-dose primidone, despite a label warning. The FDA lists a Class II recall of compounded tirzepatide syringes for sterility. The pearl covers how slowly enzyme induction wears off.

Clinical update

Bispecific antibodies differ in their non-relapse mortality, driven largely by infection

With bispecific antibodies, infection is the main driver of non-treatment deaths; monitor immunoglobulins, use prophylaxis and review whether indefinite dosing is needed.

Primary outcome
Non-relapse mortality; secondary CRS, ICANS, infection and death
Effect
Highest toxicity and NRM signals with teclistamab and glofitamab; lowest with mosunetuzumab

Hydroxychloroquine's active metabolite, set by CYP2D6, tracked lupus control better than the parent drug

Metabolite exposure, shaped by CYP2D6, may explain variable hydroxychloroquine response in lupus; it is not yet a routine test.

FDA lists Class II recall of compounded tirzepatide syringes for lack of sterility assurance

Compounded tirzepatide syringes from one US firm are under Class II recall for sterility; steer patients to licensed products.

Lenacapavir stayed in range alongside low-dose primidone in one monitored case

Lenacapavir with low-dose primidone remained therapeutic in one monitored case; if the combination is unavoidable, monitor levels and viral load.
