A wide pulmonary artery on CT says less in fibrosis, and sugammadex trimmed post-operative lung complications
In 252 patients with chronic lung disease and right heart catheterisation, CT artery size tracked pulmonary pressure well in airways disease but poorly in interstitial disease. A 3,498-patient trial found a small reduction in pulmonary complications with sugammadex.
The edition in brief
In the PVDOMICS cohort, 252 patients with chronic lung disease had CT pulmonary artery measurements compared with right heart catheter pressures; artery size was associated with pulmonary hypertension overall (AUC 0.79 for diameter) but performed worse in interstitial lung disease (PA:aorta AUC 0.70) than in obstructive disease (0.87), and the artery dilated less for the same pressure rise in fibrosis. An observational study of 1,169 adults with asthma found oscillometry-defined small airway dysfunction associated with obesity and with severe exacerbations, accounting for roughly a quarter to two-fifths of the obesity–exacerbation link. A meta-analysis of 13 studies (9,983 adults) found artificial intelligence applied to photoplethysmography detected obstructive sleep apnoea with pooled sensitivity 80% and specificity 77%, with wide uncertainty. The SNaPP trial, published in June, randomised 3,498 adults having major abdominal or thoracic surgery to sugammadex or neostigmine; pulmonary complications or death fell from 21.5% to 19.0% (RR 0.88, 95% CI 0.77 to 1.00), driven by atelectasis. The FDA recorded a supplement on 8 September to a generic triamcinolone acetonide application; the formulation and change are not specified. The pearl covers inhaler technique before escalation.
