A two-year relapse risk estimate for ANCA vasculitis, built from PEXIVAS

Six features at presentation and remission predicted first relapse in 649 trial participants, with fair discrimination and good calibration. Also: first-line JAK inhibitors and cardiovascular events in Korea, nerandomilast in autoimmune lung fibrosis, and an adalimumab biosimilar supplement.

The edition in brief

A post hoc analysis of 649 PEXIVAS participants with severe ANCA-associated vasculitis who reached remission derived a model estimating two-year risk of first relapse from sex, induction agent (oral cyclophosphamide or rituximab), ANCA subtype, non-haemorrhagic respiratory or mucosal and eye involvement, and eGFR at remission; 100 relapsed. Discrimination was fair (AUC 0.71) and calibration good, but it awaits external validation. A Korean nationwide cohort of 6,174 patients with rheumatoid arthritis found no significant difference in major cardiovascular events between first-line JAK inhibitors and TNF inhibitors (adjusted HR 0.64, 95% CI 0.28 to 1.43), with wide intervals and a population at relatively low cardiovascular risk. In 325 patients with autoimmune-related progressive pulmonary fibrosis in FIBRONEER-ILD, nerandomilast did not significantly change 52-week FVC decline in this subgroup, though time-to-event outcomes, including death, favoured the higher dose. FDA logged a supplement to the adalimumab-aqvh biosimilar licence, an administrative action. The pearl covers the cardiovascular risk review that should precede any JAK inhibitor.
