Each 5 mg of daily steroid in lupus was linked to damage risk for two years
A nationwide Swedish cohort shows glucocorticoid exposure echoes in organ damage long after the dose, pulmonary hypertension in anti-synthetase syndrome is linked to more than double the mortality, and bimekizumab's MRI effect on the spine holds at two years.
The edition in brief
In 3,749 Swedish patients with newly diagnosed lupus, each 5 mg a day of oral glucocorticoid was associated with a 42% higher hazard of first organ damage when exposure over the preceding two years was counted, and cardiovascular damage reflected dosing over three years. An international registry of 1,868 patients with anti-synthetase syndrome found pulmonary hypertension in 8.9%, more than in other myositis or rheumatoid arthritis, and independently linked to higher mortality (HR 2.52). In 102 patients with radiographic axial spondyloarthritis from BE MOBILE 2, bimekizumab reduced spinal MRI inflammation by week 16 with the effect sustained to two years and little change in erosions or new bone. A Toronto cohort of 165 patients with lupus nephritis found no detectable difference in biopsy class or activity and chronicity indices with prior antimalarial use. The pearl covers hydroxychloroquine dosing and retinal screening.
Pulmonary hypertension in anti-synthetase syndrome: common, and linked to higher mortality
Consider screening for pulmonary hypertension in anti-synthetase syndrome; it affected about 1 in 11 patients and was linked to higher mortality.
Bimekizumab's reduction in spinal MRI inflammation held at two years
Bimekizumab's reduction in spinal inflammation on MRI was sustained to two years in radiographic axial spondyloarthritis.
Prior antimalarial use did not change biopsy findings in lupus nephritis
Antimalarial use did not predict milder kidney biopsy findings in lupus nephritis; keep the usual threshold for biopsy.
Hydroxychloroquine: dose by weight, screen the retina
Keep hydroxychloroquine at or below 5 mg/kg a day and screen the retina annually after five years.
Glucocorticoid dose in lupus was linked to organ damage for years afterwards
In lupus, keep glucocorticoids as low and short as possible - each 5 mg a day was linked to 42% higher damage risk over the following two years.
