Tapering biologics in stable RA held control at 24 weeks, and ianalumab moved Sjögren activity only slightly
A randomised trial found structured tapering of biologic and targeted synthetic DMARDs non-inferior to continuation, two phase 3 trials showed a small ESSDAI benefit for ianalumab without symptom gains, and rheumatic disease was linked to more long COVID.
The edition in brief
Today's rheumatology edition. In a 24-week open-label randomised non-inferiority trial of 348 people with rheumatoid arthritis in sustained low disease activity on biologic or targeted synthetic DMARDs, low disease activity was kept by 88.2% after stepwise tapering and 89.6% on continued dosing, meeting non-inferiority, and most who flared regained control after re-escalation. Two 52-week phase 3 trials of 779 people with active Sjögren's disease found monthly ianalumab reduced ESSDAI by about 1 point more than placebo, without improving response rates or patient-reported symptoms. In a matched US cohort of 187,106 people, autoimmune rheumatic disease was associated with a 56% higher risk of coded long COVID, with rituximab and glucocorticoid exposure linked to more risk. A single-centre registry found rituximab and belimumab achieved similar rates of lupus low disease activity. The pearl covers how to taper a biologic safely.
Ianalumab lowered Sjögren activity scores slightly, but not symptoms
Ianalumab gave a small, statistically significant reduction in Sjögren activity scores but no improvement in symptoms; it does not yet change practice.
Rheumatic disease was linked to more long COVID, especially with rituximab or steroids
People with autoimmune rheumatic disease, particularly on rituximab or steroids, had more recorded long COVID; ask about symptoms after infection.
Rituximab and belimumab reached lupus low disease activity at similar rates
Rituximab and belimumab reached lupus low disease activity at similar rates in this registry; choose by clinical context and access.
Taper a biologic in steps, with a plan to step back up
Tapering works best as a stepped plan with an agreed route back to full dose.
Stepwise tapering of biologics in stable RA was non-inferior to continuing full dose
In stable RA, a structured stepwise taper of biologic or targeted synthetic DMARDs kept low disease activity as well as full dosing at 24 weeks; consider it with close follow-up.
