The ACC, AHA and seven partner societies have published a guideline on cardiovascular evaluation and management for non-cardiac surgery in JACC (29 September). It formally supersedes the 2014 document.
It covers pre-operative risk assessment, perioperative drug management, monitoring, devices and associated medical conditions. Its evidence search ran to March 2023.
Every cardiologist asked for pre-operative clearance will be held to it. Read the risk-assessment and drug-management sections before the next referral; local hospital protocols written against the 2014 version will need checking.
- The 2014 ACC/AHA perioperative guideline is formally superseded
- Review hospital pre-operative pathways that cite the 2014 document
- Check the new sections on perioperative drug management before changing practice
- Note the evidence search closed in March 2023
Why it matters
Pre-operative clearance letters written to the old guideline may no longer reflect the standard of care.
