Each DOAC has its own dose-reduction rule in atrial fibrillation. For apixaban, the reduced dose is used when two of three criteria are present: age 80 or more, weight 60 kg or less, or serum creatinine 1.5 mg/dL or more. Rivaroxaban and dabigatran reductions rest mainly on creatinine clearance, and edoxaban on clearance, weight or interacting drugs. Low body weight alone, or an age that feels old, is not a reason to drop the dose. Calculate creatinine clearance with the Cockcroft–Gault formula using actual weight, and recheck it at least yearly, more often in older or frail patients.
- Check the specific label criteria for the DOAC you prescribe before reducing the dose.
- For apixaban, two of age ≥80, weight ≤60 kg or creatinine ≥1.5 mg/dL are needed.
- Use Cockcroft–Gault creatinine clearance, not eGFR, for DOAC dosing decisions.
- Recheck renal function at least yearly and after any acute illness.
Why it matters
Off-label under-dosing is common and leaves patients exposed to stroke.
