A regulatory T-cell-expanding biologic cut eczema severity by 61%

Rezpegaldesleukin takes a different approach to atopic dermatitis from anything currently in use: it is an interleukin-2 receptor agonist that selectively expands and enhances regulatory T cells, aiming to restore immune tolerance rather than block a specific inflammatory pathway. REZOLVE-AD, a phase 2b double-blind trial at 107 centres in ten countries, randomised 393 biologic-naive adults with moderate-to-severe disease, EASI of at least 16 and at least 10% body surface area, to three dosing regimens or placebo for 16 weeks.

All three dose groups met the primary endpoint, with dose-dependent effect. Mean EASI fell 61% on 24 μg/kg every two weeks, 58% on 18 μg/kg every two weeks and 53% on 24 μg/kg every four weeks, against 31% on placebo — treatment differences of 30, 27 and 22 percentage points respectively. Injection-site reactions were the dominant adverse event, in 70% against 4% on placebo, though more than 99% were mild to moderate and resolved. There was no excess of serious adverse events and no deaths.

This is phase 2b, so the appropriate response is interest rather than expectation. Two things make it worth following: the mechanism is genuinely novel in a field increasingly crowded with cytokine blockade, and a Treg-enhancing approach could in principle produce durable tolerance rather than suppression requiring indefinite dosing. Whether that materialises will not be known from a 16-week induction period.

The statistics, in plain English

The placebo group improved 31% on its own, which is why the treatment difference of 30 percentage points is the number that matters rather than the headline 61%. All three intervals exclude zero and the dose-response ordering — 30, 27, 22 points as dosing reduces — is itself evidence the effect is real rather than chance, since a spurious finding would not usually line up with dose so neatly.
