Mesenchymal stem cells for moderate to severe atopic dermatitis: early signals from small trials

Treat mesenchymal stem cells as research-stage in atopic dermatitis and counsel patients against unregulated clinics.

Design
Systematic review and meta-analysis of four randomised and one non-randomised trial
Population
236 participants with moderate to severe atopic dermatitis
Primary outcome
EASI-50 and EASI-75 response and safety
Effect
EASI-50 at week 12 46.76% (95% CI 32.36% to 61.72%); EASI-75 at week 12 17.41% (5.56% to 43.03%)

This systematic review and meta-analysis pooled five trials, four randomised and one not, enrolling 236 participants with moderate to severe atopic dermatitis treated with mesenchymal stem cells from cord blood, autologous fat or allogeneic bone marrow.

Pooled EASI-50 response was 46.76% at week 12 (95% CI 32.36% to 61.72%). EASI-75 response was 17.41% at week 12 (5.56% to 43.03%) and 23.97% at week 16 (16.48% to 33.50%). Treatment-emergent adverse events occurred in 26.86% (19.56% to 35.68%), most often infections (7.97%) and gastrointestinal events (3.52%).

These are single-arm response proportions, not differences against placebo, and the trials were small and different in cell source and dosing. The authors call the signal early and ask for larger randomised trials. For now this is a research-stage therapy and not an option to offer outside a trial.

  • Pooled EASI-75 at week 12 was 17.4% with wide uncertainty (5.6% to 43.0%).
  • Cell source and dosing differed across the five trials.
  • Adverse events were reported in about a quarter of participants.
  • Do not offer stem cell products outside regulated trials; unregulated clinics exist.
  • Established systemic options remain the benchmark for moderate to severe disease.

Why it matters

Patients may ask about stem cell treatments they have seen advertised, and the evidence is still early.

Don't overread it

This pools small trials without a placebo-adjusted comparison, so it cannot show efficacy against standard treatments.

The statistics, in plain English

The response figures are pooled proportions of treated patients, with no control comparison, so they do not show how much of the response was due to the treatment itself. Wide confidence intervals, such as 5.56% to 43.03% for EASI-75 at week 12, reflect the small and varied trials.
