Survodutide in type 2 diabetes with obesity: about 8–10% weight loss at 76 weeks

Survodutide gave placebo-adjusted weight loss of about 4–6 points in type 2 diabetes, but it is investigational and untested against existing agents.

Design
Phase 3, randomised, double-blind, placebo-controlled
Population
752 adults with type 2 diabetes and BMI ≥27
Primary outcome
Percent change in body weight and ≥5% weight loss at week 76
Effect
−8.2% (3.6 mg) and −9.8% (6.0 mg) vs −3.9% placebo; ≥5% loss in 57.6%, 64.5% vs 35.1%

SYNCHRONIZE-2 randomised 752 adults with type 2 diabetes and a BMI of 27 or more to once-weekly survodutide, a glucagon and GLP-1 receptor dual agonist, at 3.6 mg or 6.0 mg, or placebo. Mean baseline BMI was 36.5 and HbA1c 7.4%.

At 76 weeks weight fell 8.2% with 3.6 mg and 9.8% with 6.0 mg, against 3.9% with placebo. HbA1c fell about 0.9 and 0.8 points, against 0.2 with placebo. Gastrointestinal adverse events affected roughly three quarters of those on the drug, against about two in five on placebo, mostly mild and transient.

The weight loss sits in the range seen with GLP-1 agonists in people with diabetes, who typically lose less than people without it. There is no head-to-head comparison with semaglutide or tirzepatide, so this trial cannot say whether the glucagon component adds anything for glycaemia or the liver. Survodutide is investigational and not approved anywhere.

  • Expect less weight loss with incretin-based drugs in people with type 2 diabetes than in those without it.
  • Warn patients that gastrointestinal effects are common in the first months of any incretin-based therapy.
  • Do not treat survodutide as an alternative yet: it has no approval from FDA, EMA or CDSCO.
  • Watch for its liver outcome trials, where a glucagon agonist may differ from GLP-1 agonists alone.

Why it matters

A third incretin class is approaching, and its case will rest on the liver rather than the scale.

The statistics, in plain English

The estimates use a treatment-regimen approach, counting everyone regardless of whether they stopped the drug, which gives a smaller but more realistic figure than on-treatment analyses. The placebo group's 3.9% loss shows how much comes from trial participation alone.
