Retatrutide cut sleep apnoea severity well beyond placebo, and a remission formula misses youth with obesity
Triple-agonist data in obstructive sleep apnoea, a recall of compounded semaglutide and tirzepatide, a sex gap in type 1 diabetes mortality, and why IDAA1c ≤9 under-calls remission in heavier children.
The edition in brief
Two nested phase 3 trials (TRIUMPH-1-OSA and TRIUMPH-2-OSA, NEJM, 21 October) found that once-weekly retatrutide reduced the apnoea-hypopnoea index by about 14 to 24 more events per hour than placebo over 80 weeks in adults with moderate-to-severe obstructive sleep apnoea and a BMI of 27 or more, with smaller effects in those with type 2 diabetes. Retatrutide is investigational and not approved anywhere. The FDA enforcement list carries Class II recalls of compounded semaglutide and tirzepatide (some combined with cyanocobalamin) from two US compounders for lack of assurance of sterility. An Israeli registry cohort of 4,299 people diagnosed with type 1 diabetes before 35 found mortality about 3.7 times that of matched controls, with a larger relative excess in women, and a smaller excess for those diagnosed after 2010. A SEARCH for Diabetes in Youth analysis found the widely used IDAA1c ≤9 definition of partial remission had 61% sensitivity overall and 45% in youth with overweight or obesity, so it under-calls preserved beta-cell function in heavier children. The pearl covers screening for sleep apnoea in people with type 2 diabetes and obesity.
Retatrutide cut the apnoea-hypopnoea index by 14 to 24 events an hour more than placebo
Retatrutide is not available, but the size of this effect strengthens the case for treating obesity as part of sleep apnoea care, alongside CPAP rather than instead of it.
FDA lists Class II recalls of compounded semaglutide and tirzepatide for sterility concerns
Ask patients using compounded GLP-1 or dual-agonist injections where they came from, and steer them towards approved, licensed products.
Type 1 diabetes still carried 3.7 times the mortality of matched peers, and the excess was larger in women
Treat young women with type 1 diabetes as carrying a relative mortality excess at least as great as men's, and review cardiovascular risk and hypoglycaemia safety in both.
Screen for sleep apnoea in type 2 diabetes with obesity
Ask a short sleep apnoea screen, such as STOP-Bang, in people with type 2 diabetes and obesity, and refer for a sleep study when it is positive.
IDAA1c ≤9 missed more than half of partial remissions in youth with overweight or obesity
Do not use IDAA1c ≤9 alone to rule out partial remission in a heavier child or teenager with new type 1 diabetes; consider stimulated C-peptide where the decision matters.
