FDA lists Class II recalls of compounded semaglutide and tirzepatide syringes

Ask where your patients get their GLP-1 injections; compounded syringes from one US firm are being recalled over sterility.

The FDA enforcement report carries a series of Class II recalls from a US compounding firm covering semaglutide–cyanocobalamin and tirzepatide–cyanocobalamin injections supplied in pre-filled syringes. The stated reason is lack of assurance of sterility, linked to the formulation and the processing equipment. The recall status is listed as ongoing.

A Class II recall means use of the product may cause temporary or reversible harm, or that serious harm is unlikely. It is a manufacturing-quality action, not a finding about semaglutide or tirzepatide as medicines.

These products are US compounded preparations. Whether any have reached Indian patients is unknown, but patients who buy GLP-1 agonists online or abroad are increasingly common, and compounded vitamin-B12 combinations are a marker of that supply chain.

  • Ask patients on a GLP-1 or GIP/GLP-1 agonist where they obtain it, especially if the product is a combination with vitamin B12.
  • Treat injection-site redness, swelling or fever in a patient using a compounded product as possible contamination.
  • This recall concerns sterility of compounded syringes, not the safety of licensed semaglutide or tirzepatide.
  • Advise patients to use licensed, pharmacy-dispensed products where they are available.

Why it matters

Demand for these drugs has created a grey supply, and sterility failures are its predictable hazard.

Don't overread it

A sterility recall of compounded product says nothing about the efficacy or safety of the licensed medicines.
