FDA class II recall: compounded tirzepatide vials lacking sterility assurance

Ask patients on injectable weight-loss drugs where they buy them, and suspect the product if injection-site infection appears.

The FDA enforcement report lists a class II recall, dated 14 October 2026, of compounded tirzepatide injection 18 mg/mL in 4 mL multi-dose vials from a US compounding pharmacy, on grounds of lack of assurance of sterility. The recall is ongoing. The same report carries separate class II recalls of compounded tirzepatide and semaglutide combined with cyanocobalamin from another compounder, for the same reason.

A class II recall means use could cause temporary or medically reversible harm, or that serious harm is remote. These are compounded products, not the licensed manufacturer's pens.

The India angle is indirect: compounded incretins are not a recognised supply route here, but patients increasingly buy injectable weight-loss products online or abroad. Ask where the product came from.

  • Ask every patient on a GLP-1 or dual agonist where they obtain it.
  • Treat injection-site infection or fever in a patient using a compounded incretin as a possible product problem.
  • Report suspected contamination to the national pharmacovigilance programme.
  • Licensed manufacturer pens are not part of this recall.

Why it matters

Demand for incretins has created an unlicensed supply chain whose quality nobody checks for you.

Don't overread it

A sterility-assurance recall is a manufacturing finding, not a report of patient harm.
