Capillary refill time is only useful if it is measured the same way each time. Variation in site, pressure, duration and lighting makes serial readings hard to compare.
A common standard is firm pressure on the pad of the index finger for about 10 seconds, with the hand at heart level, timing return of colour with a watch rather than counting by eye.
- Use the same finger and site for each reading.
- Apply firm pressure for about 10 seconds, with the hand at heart level.
- Time refill with a watch or phone rather than estimating.
- Note ambient temperature, as cold peripheries prolong refill.
Why it matters
A bedside sign is only as reliable as the way it is measured.
