Sarcopenia in Indian adults with type 2 diabetes: about one in four, with very low certainty

Watch for muscle weakness in adults with long-standing type 2 diabetes, without screening routinely.

A systematic review and meta-analysis in BMJ Open pooled seven cross-sectional Indian studies (1359 adults with type 2 diabetes) that used Asian Working Group for Sarcopenia criteria. Confirmed sarcopenia was estimated at 27.4% (95% CI 18.9–37.9%), but heterogeneity was very high (I² 91.7%) and the prediction interval ran from 6.9% to 65.6%. Estimates were lower with DXA (20.2%) than with bioimpedance (31.7%). Sarcopenic obesity was estimated at 25.2%.

The authors rate certainty as very low and say the data do not support a screening recommendation. They do support awareness: muscle loss in diabetes is easy to miss in a patient whose weight looks normal or high.

For family physicians the useful step is simple — notice weakness, slow walking or falls in patients with long-standing diabetes, and keep protein intake and resistance activity on the agenda.

  • Ask patients with long-standing diabetes about weakness, slowing and difficulty rising from a chair.
  • Grip strength or a five-times chair-stand test can be done in a consulting room.
  • Encourage resistance exercise and adequate protein, especially in older patients losing weight.
  • These estimates are too uncertain to justify routine screening.

Why it matters

Sarcopenia can hide behind a normal or high weight in Indian patients with diabetes.

Don't overread it

Very low-certainty prevalence data from seven small studies; the true figure could be anywhere from about 7% to 66%.

The statistics, in plain English

I² of 92% means the studies disagreed far more than chance would explain. The prediction interval tells you what a new Indian clinic population might show, and it is very wide.
