Read the discharge summary for the actions, not just the diagnosis

Turn every request in a discharge summary into a task with an owner and a date.

Discharge summaries often bury the requests to primary care — repeat a blood test, adjust a dose, chase a result, review in two weeks — in the middle of the text. Read each one for explicit actions, assign each an owner and date in the practice system, and code it so that it surfaces if missed.

  • Highlight every requested action on the discharge summary.
  • Give each action a named owner and a due date.
  • Flag patients with dementia or a carer for a direct call after discharge.

Why it matters

An action that nobody owns is the one that gets missed.
