Prediabetes carries a raised risk of progressing to type 2 diabetes and is also a marker of cardiovascular risk. Standard primary care practice is to recheck HbA1c or fasting glucose at least yearly, address weight, activity and diet, and treat blood pressure and lipids on their own merits.
Many patients are told they have "borderline sugar" and never recalled. A recall system matters more than any single consultation.
- Code prediabetes in the record so the patient enters a recall list.
- Recheck HbA1c or fasting glucose at least once a year.
- Assess blood pressure, lipids and smoking at the same visit.
- Give a specific activity and weight goal, and review it next time.
Why it matters
The patients who progress are usually the ones nobody called back.
