The Rome Foundation and the International Organization for the Study of IBD have issued a joint consensus on evaluating and managing inflammatory bowel disease patients with irritable bowel syndrome-like symptoms.
The clinical problem is one of the commonest in an IBD clinic and one of the least well handled. A patient with quiescent Crohn's or colitis on objective markers who continues to report pain, urgency and altered bowel habit presents a genuine diagnostic fork: escalate immunosuppression for disease you cannot demonstrate, or address a functional overlay and risk missing inflammation. Both errors are frequent, and they pull in opposite directions.
A joint statement from the two bodies that own each side of that question is more useful than either would be alone — the Rome Foundation defines the functional disorders, IOIBD defines the inflammatory ones, and the patients in question sit precisely where the two definitions meet.
