- Design
- Pilot randomised controlled trial, three arms
- Population
- 79 adults with IBS and high symptom burden
- Primary outcome
- Feasibility of personalised allocation
- Effect
- 73.4% completed therapy; no difference between personalised and mismatched allocation
PIONEER-IBS was a pilot randomised trial of 79 patients with IBS and high symptom burden (IBS-SSS around 314). After seven weeks of daily symptom diaries, patients were assigned either eye movement desensitisation and reprocessing or gut-directed hypnotherapy — matched to their symptom pattern, deliberately mismatched, or given optimised usual care.
The primary outcome was feasibility: 73.4% completed at least five of seven sessions. Somatic symptom burden improved within both therapy groups after treatment, but outcomes did not differ between personalised and mismatched allocation at the end of treatment or three months later.
Gut-directed hypnotherapy already has trial evidence in IBS. This pilot does not show that tailoring the choice of therapy to symptom diaries adds anything, and it was not designed to test efficacy.
- Offer a brain-gut psychological therapy, such as gut-directed hypnotherapy or CBT, for refractory IBS
- Explain the brain-gut model to patients before referral to improve uptake
- Do not delay therapy for symptom diaries aimed at matching a therapy type
- Continue diet and drug measures alongside psychological treatment
Why it matters
It tempers enthusiasm for personalising IBS therapy before simpler, proven options are used.
Don't overread it
This was a feasibility pilot; within-group improvements cannot show that either therapy works better than usual care.
The statistics, in plain English
Within-group improvement compares patients with themselves before and after, which also captures natural improvement and placebo effects. Only between-group comparisons test the therapy, and those showed no difference.
