Resmetirom worked as well in MASH patients carrying high-risk genotypes as in those without

A prespecified MAESTRO-NASH analysis finds no reason to genotype before prescribing. Also: a GnRH agonist for polycystic liver disease stopped early for side effects without shrinking livers, and how machine perfusion damps inflammation after liver transplantation.

The edition in brief

In a prespecified analysis of 738 genotyped, twice-biopsied patients in the phase 3 MAESTRO-NASH trial, resmetirom's effects on MASH resolution, fibrosis improvement, MRI-PDFF and biomarkers at 52 weeks were similar in carriers and non-carriers of high-risk PNPLA3, HSD17B13, TM6SF2, MTARC1 and MBOAT7 variants, alone or combined; the study was not powered for small interactions. In an open-label trial of 14 premenopausal women with severe polycystic liver disease, leuprorelin-induced menopause did not significantly slow liver growth (median 4.0% vs 11.0% a year, p = 0.15), did not improve quality of life, and caused far more adverse events (38 vs 9), so the trial was stopped. A mechanistic substudy of 47 extended-criteria liver transplants randomised to static cold storage, hypothermic oxygenated perfusion or normothermic perfusion found that both perfusion methods reduced neutrophil infiltration during early reperfusion injury.
