A short pangenotypic course works only in the population studied: treatment-naive adults without cirrhosis. Before treatment, confirm active infection with HCV RNA, since an antibody test only shows past exposure. Assess for cirrhosis with a simple score such as APRI or FIB-4, or elastography where available. Test for hepatitis B and HIV, and check for interacting drugs. Confirm cure with HCV RNA 12 weeks after the course ends.
- Confirm active infection with HCV RNA before treating.
- Exclude cirrhosis with APRI, FIB-4 or elastography.
- Screen for hepatitis B and HIV co-infection.
- Recheck HCV RNA 12 weeks after treatment to confirm cure.
Why it matters
The shorter course only applies to patients who truly match the trial population.
