The incidental thyroid nodule under 1 cm usually needs nothing more

Without high-risk features, an incidental thyroid nodule under 1 cm needs a TSH and a history, not more imaging.

Small thyroid nodules turn up on carotid Doppler, neck CT and chest CT all the time. In adults without high-risk features, a nodule under 1 cm found incidentally generally does not need a dedicated thyroid ultrasound or biopsy. Most are benign, and the small cancers found by chasing them are mainly indolent papillary microcarcinomas.

The exceptions are what to look for: a history of neck irradiation in childhood, a strong family history of thyroid cancer, palpable lymph nodes, hoarseness, or suspicious features already reported on the scan.

  • Check thyroid function; a nodule with low TSH is handled differently.
  • Ask about childhood neck irradiation and family history of thyroid cancer or endocrine neoplasia.
  • Examine the neck for lymph nodes and ask about voice change.
  • Without those features, record the finding and reassure rather than ordering further imaging.

Why it matters

Chasing small incidental nodules finds harmless cancers and leads to operations that do not help patients.
