Possible osteosarcopenia linked to lower intrinsic capacity and early cognitive change

Check bone and muscle together in older adults, and consider screening cognition when both are low.

Design
Longitudinal national household survey cohort
Population
2,291 community-dwelling older adults, Singapore
Primary outcome
Intrinsic capacity at baseline and cognition at 6 months
Effect
Possible osteosarcopenia in 50.3%; associated with lower intrinsic capacity and poorer 6-month cognition

A Singapore national household survey of 2,291 older adults defined possible osteosarcopenia as screening evidence of both osteoporosis and sarcopenia. It was present in half the cohort (50.3%).

Possible osteosarcopenia was associated with lower intrinsic capacity at baseline, including cognition, and with poorer cognitive function six months later, mainly in delayed recall and orientation. Education, social networks, resilience and quality of life were associated with better capacity and cognition.

This is observational and uses screening definitions rather than DXA and formal muscle testing, so 'possible' is the right word. But it adds to the case for checking bone and muscle together, and suggests that a frail musculoskeletal picture should prompt attention to cognition too.

  • Screen for sarcopenia with grip strength or chair rise alongside fracture risk assessment.
  • Consider cognitive screening in older adults found to have both low bone and low muscle.
  • Encourage resistance exercise and adequate protein, which address both bone and muscle.
  • Support social engagement, which was associated with better cognition.

Why it matters

It links two common, treatable conditions to early cognitive change.

Don't overread it

Screening-based definitions and six months of follow-up cannot show that osteosarcopenia causes cognitive decline.
