After nursing-home rehab, prior admissions and comorbidity predict readmission, not dementia

When planning discharge after rehabilitation, target support at patients with recent hospital use, high comorbidity or no one at home.

Design
Retrospective cohort, survey data linked to Medicare claims
Population
4,527 discharges home after short skilled nursing home rehabilitation stays
Primary outcome
30-day hospital readmission
Effect
15.8% readmitted; dementia not associated; prior use and comorbidity associated

Researchers linked US Health and Retirement Study data to Medicare claims (2000–2019) to study 4,527 discharges home after a short rehabilitative stay in a skilled nursing home.

Within 30 days, 15.8% were readmitted to hospital. A claims-based dementia diagnosis, present in 42% of discharges, was not associated with readmission. Hospital or emergency visits in the previous year and higher Charlson comorbidity scores were associated with higher odds. Being female, being married, and living in the Midwest or West were associated with lower odds.

This is observational US data, and claims-based dementia diagnoses are imperfect. But it suggests discharge planners should focus on prior hospital use, comorbidity and social support rather than assuming dementia alone marks the highest risk.

The finding on marital status points to the role of someone at home; whoever the carer is, checking who will be there after discharge is the practical step.

  • Flag patients with recent admissions or emergency visits as high readmission risk.
  • Factor in comorbidity burden when planning discharge support.
  • Ask who will be at home to help after discharge.
  • Do not assume dementia alone identifies the highest-risk patients.

Why it matters

Discharge resources are limited, and this points them at the factors that actually predicted readmission.

Don't overread it

Observational US claims data; it does not show that targeting these factors reduces readmissions.

The statistics, in plain English

A readmission rate of 15.8% means about one in six people returned to hospital within a month. 'Not associated' for dementia means no clear difference was found after adjustment, not that dementia has no effect on care needs.
