AF in Asian patients: standard-dose DOACs did better than warfarin — and better than reduced doses

In COMBINE AF, pooling individual data from the four pivotal DOAC-versus-warfarin trials, 10,212 Asian patients had lower body weight and creatinine clearance than 61,471 non-Asian patients. Standard-dose DOACs reduced stroke or systemic embolism (HR 0.65) and major bleeding (HR 0.62) more in Asian patients, without the increase in gastrointestinal bleeding seen in non-Asians; benefits held across weight and creatinine clearance. Reduced-dose DOACs raised stroke or embolism by 57% compared with standard doses. An automated anti-nephrin antibody assay agreed well with the research reference method in 121 children with minimal change disease or FSGS, detected more positives, and antibody levels in remission were associated with relapse. Whole-genome sequencing of 1,052 people with CAKUT gave a monogenic diagnosis in only 4.9%, higher with family history, consanguinity or extra-renal features. A network meta-analysis associated suction ureteroscopy sheaths with higher stone-free rates and less postoperative sepsis.
