One in three on susceptible contraception was also taking an enzyme-inducing antiseizure drug

A Medicaid cohort finds long overlaps between interacting antiseizure drugs and hormonal contraception; inebilizumab cuts myasthenia exacerbations; activity tracks slower Parkinson progression.

The edition in brief

Among nearly 22,000 US Medicaid beneficiaries with epilepsy prescribed a hormonal contraceptive vulnerable to enzyme induction, a third had at least two weeks of overlap with an enzyme-inducing antiseizure drug, with a median overlap of 218 days. Overlap was more likely with intellectual disability, drug-resistant epilepsy and age over 35. A prespecified analysis of the MINT trial found that inebilizumab, a CD19 B-cell-depleting antibody, cut the hazard of exacerbations or rescue therapy by about 60% in generalised myasthenia gravis during a steroid taper, in both AChR and MuSK antibody-positive disease. A cohort of 294 people with Parkinson disease found those in the highest third of physical activity had slower cognitive and motor decline over up to seven years, an association rather than proof. Screening data from the AHEAD 3-45 trial showed plasma p-tau217 runs lower in Asian, Black and Hispanic participants than in White participants, though its relation to amyloid PET held across racial groups. A genetic proxy for higher GLP-1 receptor expression was linked to less Parkinson disease among people starting GLP-1 drugs. The pearl covers bedside monitoring in myasthenic weakness.

Inebilizumab and myasthenia exacerbations during steroid taper

Inebilizumab reduced exacerbations during steroid taper in generalised myasthenia; consider it where approved for patients who relapse as steroids come down.

More physical activity, slower Parkinson progression

Prescribe sustained, regular physical activity in Parkinson disease; duration seems to matter more than intensity.

Plasma p-tau217 runs lower in Asian, Black and Hispanic adults

Treat plasma p-tau217 as a triage test before PET or CSF, and be cautious applying a single cut-off across populations.

GLP-1 receptor genetics and Parkinson risk on GLP-1 drugs

This is a research signal about who might benefit from GLP-1 drugs in Parkinson disease; it does not change prescribing.
