This umbrella review synthesised nine systematic reviews yielding 92 comparisons of immediate against delayed insertion of long-acting reversible contraception in postpartum and postabortion populations, assessing each review with AMSTAR-2 and grading the certainty of pooled findings.
The efficacy case is clear. Immediate insertion gave higher initiation, higher use at six months, and lower six-month pregnancy rates, with a relative risk of 0.16 (95% CI 0.04-0.71) on moderate-certainty evidence, and a similar pattern after abortion. The harm is equally clear and equally well evidenced: immediate intrauterine device insertion markedly increased expulsion, and the subgroup effect matters. After vaginal delivery the risk ratio was 5.09 (95% CI 2.04-12.73) on high-certainty evidence; after first-trimester medical abortion it was 2.02.
These findings are not in conflict, and the resolution is in the counselling rather than in the policy. The pregnancies prevented are prevented largely because a woman who leaves without a device often does not return for one, and that is a bigger effect than expulsion. So immediate insertion remains the right default, particularly where follow-up is unreliable — but a woman receiving an intrauterine device immediately after a vaginal birth should be told expulsion is several times more likely, taught to check the threads, and given a clear route back if it happens.

The statistics, in plain English
The two headline findings differ in how firmly they are established. The expulsion result carries high-certainty GRADE with an interval of 2.04 to 12.73 — the width reflects imprecision, but even the lowest value is a doubling. The pregnancy-prevention interval, 0.04 to 0.71, is also wide and rests on few events, which is why it is rated moderate rather than high. Both are real; the expulsion figure is the better-evidenced of the two.
