ACOG grades its menopause symptom recommendations

Read ACOG's graded menopause guideline in full and check each recommendation's strength before changing practice.

ACOG has issued a clinical practice guideline covering vasomotor, sleep and mood symptoms of spontaneous or induced menopause at or after age 40. A three-member writing team screened studies against set criteria, assessed quality, and used a modified GRADE evidence-to-decision framework.

Each recommendation carries a strength and an evidence-quality grade, and the guideline adds ungraded Good Practice Points where evidence was too thin for a formal recommendation. It covers both hormonal and non-hormonal drug options and non-drug approaches.

The abstract does not list the individual recommendations, so the specific advice should be read in the full guideline before it changes a protocol. Its value is a single graded reference for the symptoms that bring most midlife women to clinic.

  • Read the full guideline before revising local menopause protocols.
  • Check which recommendations are strong and which are Good Practice Points only.
  • Use it alongside existing menopause guidance for women under 40, which it does not cover.
  • Ask specifically about sleep and mood, not only hot flushes.

Why it matters

Graded recommendations let clinicians separate firm advice from expert opinion when patients arrive with conflicting online claims.
