A consensus statement backs barrier-forming topical agents for patients at high risk of severe radiation dermatitis

For patients at high risk of severe radiation dermatitis, especially in head and neck fields, consider a barrier-forming agent from the start of treatment.

Design
Clinical practice statement based on literature review and expert consensus
Population
Patients receiving radiotherapy at risk of acute radiation dermatitis
Primary outcome
Recommendations on use of barrier-forming topical agents
Effect
Recommended for high-risk patients, especially irregular sites such as head and neck

A clinical practice statement from the Multinational Association of Supportive Care in Cancer oncodermatology group reviewed the literature to April 2026 and gathered expert consensus on barrier-forming topical agents, such as film-forming gels and protective films, to prevent acute radiation dermatitis.

The statement recommends these agents for patients at considerable risk of high-grade dermatitis, particularly where the treatment field has irregular contours, such as head and neck cancer. It covers patient selection, skin preparation, product choice, application technique and monitoring. Use at other sites may be considered, but the authors say the evidence there is not yet strong enough for routine use. One film-forming product has the most supporting data.

Clinical practice statements are consensus documents written where high-quality evidence is limited, so they guide rather than mandate. Cost and availability of specific products in India vary.

  • Consider a barrier-forming agent from the start of radiotherapy for patients at high risk of severe skin reactions
  • Prioritise head and neck fields and other irregular contours
  • Follow product-specific guidance on skin preparation and application timing
  • Continue to assess the skin weekly during treatment and for two weeks after
  • Keep general skin care advice: gentle washing, loose clothing, avoid irritants

Why it matters

Severe radiation dermatitis can interrupt treatment, and prevention is easier than repair.

Don't overread it

This is an expert consensus statement where high-quality evidence is limited, not a formal guideline.
