Sarcopenia predicted death in liver cancer across all treatments

Consider body composition from the staging scan as part of risk assessment in hepatocellular carcinoma, and refer sarcopenic patients for nutrition support.

Design
Systematic review and meta-analysis of observational studies
Population
92 studies of patients with hepatocellular carcinoma
Primary outcome
Overall survival by body composition
Effect
Sarcopenia pooled HR 1.85 (95% CI 1.75–1.94; I² 0%)

This systematic review included 92 studies of patients with hepatocellular carcinoma who had body composition measured on CT, MRI or bioimpedance, across curative, locoregional and systemic treatment.

Sarcopenia was common, with a median study-level prevalence of 38% to 50%. In a meta-analysis of 44 studies it was associated with worse overall survival (pooled HR 1.85, 95% CI 1.75–1.94, I² 0%), with similar effects in each treatment group, and with recurrence after curative treatment (HR 1.71). Sarcopenic obesity carried the highest risk after curative treatment. In systemic therapy, fat infiltration of muscle predicted mortality while visceral fat appeared protective.

Muscle mass can be measured from the staging scan already done. Whether improving it changes outcomes has not been shown, but it identifies patients who need nutrition and activity support.

  • Ask radiology to report muscle mass at L3 on staging scans where possible.
  • Flag sarcopenia as a marker of higher risk when discussing treatment options.
  • Refer sarcopenic patients for nutritional assessment and supervised activity.
  • Do not treat a normal BMI as reassurance; sarcopenic obesity carried the highest risk after curative treatment.

Why it matters

A routine scan already holds a strong prognostic marker that is rarely reported.

Don't overread it

This does not show that treating sarcopenia improves survival.

The statistics, in plain English

A pooled hazard ratio of 1.85 means about 85% higher risk of death over time. I² of 0% means the studies agreed closely. These are observational associations, so sarcopenia may mark frailer patients rather than cause the worse outcomes.
