Tandem CAR T-cells more than doubled event-free survival in transplant-ineligible lymphoma

For transplant-ineligible relapsed large B-cell lymphoma, CAR T-cell therapy now has randomised support; refer early for assessment.

Design
Open-label randomised phase 2 trial
Population
160 transplant-ineligible adults with relapsed or refractory large B-cell lymphoma, median age 74
Primary outcome
Event-free survival by blinded independent review
Effect
6.2 vs 2.5 months; HR 0.39 (95% CI 0.27–0.58)

DALY 2-EU, an open-label randomised phase 2 trial at 45 European centres, enrolled 160 adults with relapsed or refractory large B-cell lymphoma after first-line chemoimmunotherapy who were ineligible for transplantation. Median age was 74. Patients received a single infusion of zamtocabtagene autoleucel — fresh, non-cryopreserved CAR T-cells targeting both CD20 and CD19 — or up to eight cycles of rituximab, gemcitabine and oxaliplatin (R-GemOx).

Median vein-to-vein time was 15 days. At a median 17 months, event-free survival, judged by a blinded committee, was 6.2 months with CAR T-cells against 2.5 months with R-GemOx (HR 0.39). Severe adverse events were common in both arms; cytokine release syndrome (8%) and neurotoxicity (5%) occurred only with CAR T-cells. There were two treatment-related deaths with CAR T-cells and one with R-GemOx.

Anti-CD19 CAR T-cell therapy is standard second-line treatment for fitter patients, but older patients who cannot have a transplant have had no randomised evidence. This trial gives it. Short manufacturing time with a fresh product also matters for patients whose disease progresses quickly.

It is a phase 2 trial with event-free survival as the end point; overall survival data are immature. Access in India is limited by cost and manufacturing, though locally developed CAR T-cell products are now approved.

  • Refer transplant-ineligible patients with relapsed large B-cell lymphoma to a CAR T-cell centre for assessment early.
  • Do not exclude older patients from CAR T-cell discussion on age alone; the median age here was 74.
  • Counsel on cytokine release syndrome and neurotoxicity and the need to stay near the treating centre.
  • Consider R-GemOx or other options where CAR T-cell therapy is not accessible.

Why it matters

It extends CAR T-cell therapy, with randomised evidence, to older patients previously limited to palliative chemoimmunotherapy.

Don't overread it

Phase 2 with immature survival data; it is not yet proof of longer life.

The statistics, in plain English

A hazard ratio of 0.39 means about 60% lower risk of an event (progression, new treatment or death) at any time. The confidence interval (0.27 to 0.58) is well clear of 1. Event-free survival is not overall survival, which remains to be reported.
