Anatomic shoulder replacement holds at 15 years, colchicine fails in knee osteoarthritis, and shockwave helps some tendons more than others
A 791-shoulder cohort reports 93.7% survivorship at 15 years with gradual functional decline, a placebo-controlled trial finds no pain benefit from colchicine in knee osteoarthritis, and a network meta-analysis finds shockwave therapy useful in some tendinopathies but not patellar.
The edition in brief
A retrospective cohort of 791 anatomic total shoulder arthroplasties reported implant survivorship of 96.1% at ten years and 93.7% at fifteen, with Walch B glenoid morphology and younger age linked to revision. At ten years 79.9% of shoulders met the minimal clinically important difference on the ASES score and 55.8% the substantial clinical benefit threshold, though function attenuated gradually. In CLOAK, 120 people with knee osteoarthritis randomised to colchicine or placebo for three months showed no difference in pain, function or effusion size, despite improvements in some inflammatory biomarkers. A phase 2 trial of 120 patients found intravenous lidocaine did not significantly reduce rebound pain after interscalene block for rotator cuff repair (23.3% versus 36.7%, relative risk 0.64, 95% CI 0.36 to 1.12), though the trial was probably underpowered. A registry-based projection estimates revision hip and knee arthroplasty volumes in 17 countries will rise about 36% by 2060. A network meta-analysis of 65 trials suggests shockwave therapy improves pain in several tendinopathies but showed no significant effect over placebo in patellar tendinopathy.
Anatomic total shoulder arthroplasty at 10 to 15 years: durable implant, gradually fading function
When consenting for anatomic shoulder replacement, describe both implant survival and the chance of lasting functional benefit, and discuss higher revision risk in younger patients and Walch B shoulders.
Colchicine for knee osteoarthritis: no gain in pain, function or effusion at three months
Do not start colchicine for knee osteoarthritis pain on current evidence; this trial found no clinical benefit at three months.
Intravenous lidocaine and rebound pain after interscalene block: inconclusive in an underpowered trial
Treat intravenous lidocaine for rebound pain as unproven; discuss it with the anaesthesia team only as a research question.
Revision hip and knee arthroplasty volume projected to rise about 36% by 2060
Use this to support arguments for registries, infection prevention and revision training, not as a forecast for your own unit.
Ask patients what they expect before any elective joint operation
Record the patient's main functional goal before elective joint surgery and revisit it at follow-up.
Shockwave therapy for tendinopathy: useful in some tendons, not in patellar tendinopathy
Consider shockwave as an adjunct for lateral epicondylitis, Achilles or rotator cuff tendinopathy where exercise alone has not helped, and avoid it for patellar tendinopathy on this evidence.
