The MT-RNR1 guideline has a use in any ENT clinic that prescribes or advises on aminoglycosides, including some ear drops: take a moment to ask whether a blood relative went deaf after an antibiotic injection. Because the trait is maternally inherited, a single such story in the maternal line is a red flag for an MT-RNR1 variant, and a reason to test or to choose a different agent. It is one question that can prevent a lifetime of avoidable, irreversible hearing loss — and it costs nothing to ask.
