- Design
- Single-centre retrospective case series
- Population
- 1,734 children undergoing intracapsular tonsillotomy with adenoidectomy
- Primary outcome
- Operating time, postoperative bleeding and revision surgery
- Effect
- Time 22 vs 24 min; bleeding 1% vs 0.9% (OR 1.1, 95% CI 0.35 to 3.47); revision 0.5% vs 1%
A retrospective series from a US children's hospital compared 1,734 intracapsular tonsillotomies with adenoidectomy performed between 2018 and 2024: 385 by coblation and 1,349 by microdebrider. Mean age was about 5 years. Published in Otolaryngology–Head and Neck Surgery on 29 September 2026.
Median operating time was 22 minutes with coblation and 24 with the microdebrider. Postoperative bleeding (1% vs 0.9%; OR 1.1, 95% CI 0.35 to 3.47) and revision surgery (0.5% vs 1%; OR 0.85, 0.23 to 3.15) did not differ significantly. Younger age was associated with revision.
Coblation was used by only two surgeons and the microdebrider by thirteen, so technique is entangled with surgeon. Events were rare, so the intervals are wide. The useful message is that both techniques gave low bleeding and revision rates for intracapsular surgery, and that younger children should be counselled about regrowth.
- Offer intracapsular tonsillotomy with either coblation or microdebrider according to local skill
- Counsel parents of younger children that tonsil regrowth and revision are more likely
- Expect postoperative bleeding of about 1% after intracapsular surgery in this series
- Audit your own bleeding and revision rates against these figures
Why it matters
Instrument choice matters less than the shift to intracapsular technique itself.
Don't overread it
Surgeon and technique are confounded, and rare events give wide intervals; no difference does not prove equivalence.
The statistics, in plain English
The bleeding odds ratio of 1.1 has an interval from 0.35 to 3.47, so the data fit anything from a third of the risk to three times the risk. With about 1% bleeding, much larger studies would be needed to separate the techniques.
