Acquired velopharyngeal insufficiency in adults usually improved with treatment

Refer adults with acquired velopharyngeal insufficiency for assessment - most improved with treatment, usually injection pharyngoplasty.

Design
Systematic review and meta-analysis of 11 studies
Population
107 adults with acquired, non-cleft velopharyngeal insufficiency
Primary outcome
Improvement in speech and swallowing
Effect
Speech improved 88% (78-94%); swallowing 78% (49-93%)

A meta-analysis in The Laryngoscope (15 October 2026) pooled 11 studies of 107 adults with non-cleft, acquired velopharyngeal insufficiency, mostly after head and neck cancer treatment (45%) or from neurological disease (47%).

Injection pharyngoplasty was the commonest treatment (82% of 163 interventions), followed by palatal lift prosthesis and surgical reconstruction. Across all treatments, speech improved in 88% (95% CI 78-94%) and swallowing in 78% (49-93%). Injection pharyngoplasty had an adverse event rate of about 5%.

The evidence is small and heterogeneous, but it supports referral: adults with hypernasal speech or nasal regurgitation after cancer treatment or neurological illness often have a treatable problem.

  • Ask about hypernasal speech and nasal regurgitation after palate or pharynx cancer treatment
  • Refer suspected acquired velopharyngeal insufficiency for speech assessment and nasendoscopy
  • Consider injection pharyngoplasty for small to moderate gaps
  • Involve speech and language therapy in every case

Why it matters

A disabling problem that is often accepted after cancer treatment appears to respond well to simple procedures.

The statistics, in plain English

Pooled proportions from small, uncontrolled studies: the 88% speech improvement has a fairly tight interval, but the swallowing estimate (49-93%) is very uncertain. Without comparison groups, some improvement may reflect natural recovery.
