Get a clean urine sample before the first antibiotic dose in a febrile infant

Collect a reliable urine sample for culture before giving the first antibiotic to a febrile child with suspected UTI.

In a febrile infant or young child in whom UTI is suspected and antibiotics are about to start, the urine sample must come first. Once antibiotics are given, a culture can be falsely negative and the diagnosis may never be confirmed, which affects decisions about imaging and future episodes.

Bag samples are acceptable for a screening dipstick but are frequently contaminated, so a positive bag result should be confirmed with a catheter or suprapubic sample before treatment where possible. A clean-catch sample is a reasonable alternative in a child who is not unwell.

  • Collect urine before the first antibiotic dose whenever the child's condition allows.
  • Confirm a positive bag specimen with a catheter, suprapubic or clean-catch sample before treating.
  • Send every sample for culture, not only dipstick.
  • Record how the sample was collected; it affects how the result is read.

Why it matters

An unconfirmed UTI leaves every later imaging and recurrence decision uncertain.
