The AAP's 2026 UTI definition now requires pyuria

Require evidence of inflammation such as pyuria, not just a positive culture, before labelling a young child with UTI.

This technical report explains how the American Academy of Pediatrics' 2026 guideline panel redefined urinary tract infection for children aged 8 days to 5 years. There is no gold standard for diagnosis, so the definition rests on three domains: symptoms, signs of inflammation and culture.

The panel made two main changes. Pyuria is now a diagnostic criterion in most clinical situations, with separate thresholds depending on age and whether culture results are available or still pending. The threshold for a positive culture was revised according to age and the method of urine collection.

The aim is to reduce overdiagnosis and unnecessary antibiotics, particularly from contaminated or bag samples, without missing true infections. Clinicians should check the guideline itself for the exact thresholds before applying them.

  • Look for pyuria as well as a positive culture before diagnosing UTI in a young child.
  • Interpret culture results according to the child's age and how the urine was collected.
  • Use catheter or suprapubic samples where possible in infants who are not toilet-trained.
  • Treat a positive culture without pyuria with caution; it may be contamination or bacteriuria.
  • Check the guideline's age-specific thresholds before changing local protocols.

Why it matters

Culture-only diagnosis drives antibiotic overuse, imaging and parental worry in children who do not have a UTI.
