Adding family-based behavioural treatment in primary care cut children's relative BMI by a further 3.8 points

TEAM UP on childhood obesity, how long to wait before stopping antiseizure drugs, follow-up within a week of PICU discharge, and why subthreshold phototherapy is so hard to stop.

The edition in brief

TEAM UP, a pragmatic trial in 730 children aged 6 to 15 with obesity across 41 US primary care practices, found that adding family-based behavioural treatment to enhanced paediatrician-led care reduced percent of median BMI by 6.4 points against 2.6 with enhanced care alone at 12 months (difference −3.8). A meta-analysis of 22 studies (3,933 patients) found relapse after stopping antiseizure medication was more likely with under five years of seizure freedom (RR 1.49), and in children with under three years (RR 1.70). A US claims study of 180,516 PICU admissions found that a primary care visit within seven days of discharge was associated with fewer emergency visits and readmissions over the next three months (IRR 0.93 public, 0.70 private insurance). A mixed-methods analysis from a US quality collaborative found subthreshold phototherapy for neonatal jaundice persisted despite pathways, weekend follow-up and home phototherapy, driven by worries about follow-up, readmission and habit. The pearl covers using hour-specific thresholds before starting phototherapy.

Clinical update

Stopping antiseizure medication after under three years seizure-free was linked to more relapse in children

When discussing withdrawal of antiseizure medication in a child, a seizure-free period of at least three years appears to lower relapse risk; individualise by epilepsy type and EEG.

Under 5 years RR 1.49 (1.17 to 1.88); children under 3 years RR 1.70 (1.20 to 2.39)

Subthreshold phototherapy persisted despite pathways, weekend follow-up and home phototherapy

Before starting phototherapy below threshold, name the reason; if it is worry about follow-up, arrange the follow-up instead.

Family-based behavioural treatment added to paediatric care reduced relative BMI more than enhanced care alone

For children with obesity, offer family-based behavioural treatment alongside clinic follow-up where it can be delivered, involving a parent in every session.
