Level-guided anti-thymocyte globulin dosing improved graft-versus-host-free survival; cenobamate raises or lowers brivaracetam depending on dose
A phase 3 trial of therapeutic drug monitoring leads an edition on dose-dependent interactions, a pharmacovigilance signal that is hypothesis only, and a two-drug HIV switch regimen chosen to avoid interactions.
The edition in brief
In a randomised phase 3 trial of 204 patients having haploidentical stem cell transplantation, anti-thymocyte globulin dosed to a monitored exposure target gave higher one-year graft-versus-host-disease-free and relapse-free survival than a fixed 10 mg/kg dose (66.7% vs 50.0%; HR 0.666, 95% CI 0.4456 to 0.9954), with less moderate to severe chronic graft-versus-host disease (9.8% vs 22.5%) and fewer grade 3 to 5 infections (44.1% vs 70.6%). A retrospective study of 891 samples from 213 patients found cenobamate had a biphasic effect on brivaracetam concentrations: about 32% higher at 12.5 to 50 mg a day and about 21% lower at 250 to 450 mg a day. An unsupervised machine-learning analysis of 40,127 fluoroquinolone reports in FAERS found a multi-system adverse-event cluster in 10.1% of reports; the authors say this is strictly hypothesis-generating. DORAL, a phase 2 trial in 114 virologically suppressed adults, found one virological failure with a once-daily doravirine plus raltegravir switch. A regulatory entry notes an FDA supplement to a generic exenatide application. A pearl on CYP3A4 inhibitors with opioids closes the edition.
FDA records a supplement to a generic exenatide application
No prescribing change follows from this record; check the product label if you use synthetic exenatide.
Cenobamate and brivaracetam: raised levels at low doses, lowered at high doses
Consider monitoring brivaracetam levels and symptoms when cenobamate is started or titrated, since the effect changes direction with dose.
DORAL: doravirine plus raltegravir, once daily, held suppression in 114 adults
A once-daily doravirine plus raltegravir switch is a plausible option for selected suppressed patients in whom interactions matter; discuss with an HIV specialist.
Machine learning found a multi-system fluoroquinolone cluster in FAERS reports: hypothesis only
Treat this as a methods paper: report suspected multi-system reactions, and keep fluoroquinolone counselling as it is.
Opioids and CYP3A4 inhibitors: ask what else was started
When a patient on oxycodone or fentanyl starts or stops any enzyme-active drug, check for a CYP3A4 interaction.
Dosing ATG to monitored exposure improved GVHD-free, relapse-free survival in a phase 3 trial
Exposure-guided ATG dosing is worth discussing with transplant and pharmacology teams where monitoring is available; the primary result was borderline.
