Gepotidacin outperformed nitrofurantoin most clearly when the uropathogen was nitrofurantoin non-susceptible

A pooled subgroup analysis of EAGLE-2 and EAGLE-3 shows where the newer oral antibacterial adds most. Also: how lithium's adverse-reaction profile in children differs from antipsychotics, an oral GLP-1 agonist that ignores mealtimes, and another compounded tirzepatide recall.

The edition in brief

A Clinical Infectious Diseases analysis, published on 28 October, pooled the EAGLE-2 and EAGLE-3 trials of gepotidacin 1,500 mg twice daily against nitrofurantoin 100 mg twice daily for five days in women and girls aged 12 or over with uncomplicated UTI. Therapeutic success favoured gepotidacin by 9.6 points with nitrofurantoin-susceptible organisms and by 20.8 points with non-susceptible ones, driven by microbiological rather than symptomatic response; subgroups at higher resistance risk showed similar or better results. It is industry-sponsored subgroup work, and stewardship should decide its place. A VigiBase case-control analysis of nearly 30,000 lithium reports found renal, endocrine, gastrointestinal, skin and cardiac reactions were proportionally more often reported in children than with second-generation antipsychotics, thyroid reactions especially. A phase 1 study of elecoglipron, an investigational oral small-molecule GLP-1 agonist, found food did not meaningfully change exposure and the half-life of about 21 hours supports once-daily dosing. The FDA lists a Class II recall of compounded tirzepatide multi-dose vials from Apothecary Pharma for lack of sterility assurance. The pearl covers lithium monitoring and interactions.

Lithium in children: renal, thyroid and gut reactions reported proportionally more than with antipsychotics

When using lithium in children, monitor thyroid and renal function closely, and set its profile against the metabolic effects of antipsychotics.

FDA lists Class II recall of compounded tirzepatide multi-dose vials

Ask where incretin injections come from; compounded multi-dose vials carry sterility risks the licensed pens do not.

Elecoglipron: an oral GLP-1 agonist whose exposure did not depend on food

Small-molecule oral GLP-1 agonists such as elecoglipron may be taken without regard to food, unlike oral semaglutide; this one remains investigational.

Gepotidacin's advantage over nitrofurantoin was largest when the organism was not nitrofurantoin-susceptible

Where gepotidacin is available, consider it for uncomplicated UTI with nitrofurantoin non-susceptible organisms, and keep first-line agents for susceptible infections.
