- Design
- Systematic review and random-effects network meta-analysis of 27 randomised trials
- Population
- Patients without baseline heart failure; 702,584 patient-years
- Primary outcome
- Major adverse cardiovascular events
- Effect
- Candesartan OR 0.79 (95% CI 0.63–0.97); telmisartan OR 0.90 (0.80–0.99) vs placebo
This network meta-analysis pooled 27 randomised trials covering 702,584 patient-years of ARB treatment in people without heart failure at baseline. Each trial had at least 100 patients per arm and a year of follow-up.
Candesartan reduced major adverse cardiovascular events against placebo (OR 0.79) and ranked highest. Telmisartan showed a smaller significant reduction (OR 0.90). Other ARBs did not show significant reductions in this network.
ARBs are often treated as interchangeable, with choice driven by cost and once-daily dosing. They do differ in receptor binding, half-life and duration of action, and this analysis suggests those differences may matter for outcomes. Both candesartan and telmisartan are widely available as generics in India.
The caveat is real: most comparisons between ARBs were indirect, and the network was sparse in places. This is a ranking, not a head-to-head trial, and it does not justify switching a patient who is well controlled on another ARB.
- Consider candesartan or telmisartan as first choice when starting an ARB for cardiovascular risk reduction.
- Leave patients well controlled on another ARB on their current drug unless there is another reason to switch.
- Use doses shown in outcome trials rather than the lowest available strength.
- Monitor potassium and creatinine after starting or changing any ARB.
Why it matters
It challenges the habit of treating all ARBs as interchangeable for outcomes.
Don't overread it
Mostly indirect comparisons; this does not prove one ARB beats another head to head.
The statistics, in plain English
An odds ratio of 0.79 for candesartan means about 21% lower odds of a major cardiovascular event than placebo; the confidence interval (0.63 to 0.97) is wide but excludes no effect. SUCRA is a ranking score from 0 to 1; 0.85 means candesartan ranked near the top. Rankings from indirect comparisons are less reliable than direct trials.
