Levetiracetam in cooled neonates: clearance tracked renal function and serum sodium

In cooled neonates with encephalopathy, factor renal function into levetiracetam maintenance dosing.

Design
Prospective population pharmacokinetic study
Population
15 term neonates with hypoxic-ischaemic encephalopathy under therapeutic hypothermia
Primary outcome
Levetiracetam pharmacokinetic parameters and covariates
Effect
Clearance 4.27 L/h (15% IIV); rose with creatinine clearance and sodium

LEVNEONAT-1 measured levetiracetam pharmacokinetics in 15 term neonates treated for seizures due to hypoxic-ischaemic encephalopathy during therapeutic hypothermia. Babies received a loading dose of 30 to 60 mg/kg, then a quarter of that dose every eight hours.

A two-compartment model fitted the data. Typical clearance was 4.27 L/h scaled to body weight. Clearance rose with creatinine clearance and with serum sodium. There was no relationship between early drug exposure and the reduction in seizure burden.

Levetiracetam is increasingly used first or second line in neonatal seizures. This small study supports adjusting maintenance dosing for renal function in cooled babies, and raises sodium as a possible marker of encephalopathy severity. The lack of an exposure–response link in 15 babies says little either way about efficacy.

  • Consider renal function when setting levetiracetam maintenance doses in cooled neonates.
  • Note that hyponatraemia was associated with lower clearance and higher exposure.
  • Do not infer efficacy from this study; it was too small to test it.
  • Follow local neonatal seizure protocols for drug choice.

Why it matters

Cooling and kidney injury both slow drug clearance in the babies most likely to need anticonvulsants.

Don't overread it

Fifteen babies; it describes kinetics, not whether levetiracetam works.

The statistics, in plain English

Population pharmacokinetic models estimate typical values and how much they vary between babies. With 15 babies, covariate findings such as sodium are early signals that need confirming.
